Care Continuity in palliative care

Project objectives

This project included several studies regarding continuity of care during the end of life, particularly regarding theoretical, organizational and assessment issues. The end of life is an extremely difficult time for patients and their families. To ensure comfort and meet all needs, it is essential to pay attention to continuity of care. The concept of “continuity of care at the end of life” has not been fully explored; then a conceptual model was created, starting from an analysis of the concept. The study continued with the exploration of the model to better understand which operational characteristics can guarantee continuity of care for those suffering from a life-limit diseaseing (multidisciplinary team, periodic evaluation) and which elements can influence its implementation (care transitions).
