Retrospective study of CKD reporting and medication prescribing/dosing in older patients discharged from geriatric and nephrology unit in Italy

Project objectives

Objective 1 – Prevalence of omitted diagnosis of CKD in Geriatrics Units; 

Objective 2 - Prevalence of inappropriate prescription/dosage of renally eliminating drugs at the time of discharge in the Nephrology and Geriatrics Unit

Objective 3 - Prevalence of failure to prescribe potentially appropriate drugs at the time of discharge in the Nephrology and Geriatrics unit.
